The Process for Clients

Step 1:  Fax or Email Order

Step 2:  Pay Fee

Step 3:  Sign and Send in ALL Notarized Forms.
FORMS THAT REQUIRE A NOTARY:

• Form-1) Informed Consent For Child Custody 2022 – Example (Signed by anyone 18+ in the home.) *MUST BE NOTARIZED
• Form-2) Consent for Release 2023 (ALL subject children and adults should be listed on this form.) *MUST BE NOTARIZED
• Form-3) CPS Form 2023  (Signed by anyone 18+ in the home.) *MUST BE NOTARIZED
• Form-4) Credit Card Guarantee 2023*MUST BE NOTARIZED (attached)
  

Step 4:  Call or Email Office and Set an Appointment.
The office will ONLY set up your appointment after payment has been made in full AND appropriate notarized paperwork has been received (Numbers 1 -3 Above).
Please note: Appointments are available at Noon on Monday, Wednesday, and Friday ONLY.

Step 5:  Complete Personal Data Form (filled out by anyone 18+ in the house)

Step 6:  Request for Letters of Reference (5 Total on Behalf of ALL Adults in Home – ONLY 2 Family Member References per Adult).

Step 7:  Set an Appointment to have your FBI Criminal Histories Pulled via Fingerprinting (To be Performed by Anyone 18+ in Home for Custody; 14+ for Adoption).
